POSITION SUMMARY

Under the supervision of the Property Manager and in close collaboration with onsite staff, resident councils, and residents, this role is responsible for developing, coordinating, and delivering a range of community-informed services that facilitate housing stabilization and promote residents’ financial, social, and health related needs. The Community Support Coordinator is responsible and accountable for delivering additional services to residents to help maintain housing stability. The Community Support Coordinator role is on the Property Operations Team but will also work in collaboration with the Supportive Services Team.

ESSENTIAL FUNCTIONS

Community Engagement

● Promote resident health and wellness through coordination of trauma-informed services with local providers and partners including but not limited to medical/mental/cognitive health, mindfulness and well-being, and substance dependency

● Develop and implement a monthly newsletter for community news and a monthly calendar of social events

○ Daily activities for both buildings 3x a week

■ Games & Activities

■ Holiday-themed activities

○ Monthly Speakers/Presentations for each building

○ Monthly Community Event

■ Meet with the property manager to ensure any community event is within budget

Resident Council

● Work in conjunction with resident council to provide social activities for residents onsite including coordinating monthly birthday celebrations and holiday celebrations

● Meet with both resident councils to ensure your activities do not pose scheduling conflicts

● Have monthly meetings with each council to ensure positive relationships between them and staff

● Attend all resident meetings when possible.

● Work collaboratively with the councils for their annual events Resident Meetings

● Meet with residents to assist them with annual recertification application paperwork if needed, help renew their social security benefits, and ensure you are getting ROI’s

● Refer residents to resources when necessary

● Alert staff when intervention or a demand is necessary

○ Meet with residents with housekeeping and hoarding issues to help learn new habits

○ Connect residents to in-home aid services when needed

○ Call APS when appropriate

○ Coordinate with the Assistant Manager to do family calls about residents when situations arise.

● New resident move-in letters Community Gardens

● Manage the assignment and monitoring of the annual garden plots and pots

● Administer the community’s gardens policy, procedures, and schedules Partnerships

● Build community partnerships to bring in activities, donations, etc.

● Meet with the Senior Center monthly to get their calendars to promote their field trips Resident Surveys

● Create and collect an annual survey for the residents to gauge the pulse of the property

● Conduct crisis and risk assessments in consultation with property operations colleagues and supervisors; support with crisis intervention services, as appropriate

● Develop and administer a needs assessment survey with residents to develop appropriate programming and services for residents

  • Ability to efficiently perform job responsibilities with minimal supervision.
  • Ability to speak concisely and effectively communicate.
  • Must be able to lift up to 30 pounds.

QUALIFICATIONS

A. Education: Highschool diploma or equivalent

B. Experience:

  • At least 2 years of experience in direct service coordination in a human services field, senior services, and/or case management is required. With excellent references, this can include documented volunteer work in the community or an unpaid internship.
  • Familiarity with or willingness to learn Google Suite applications.
  • Bilingual proficiency in a common language other than English is preferred but not required.
  • Experience working with older adults is strongly preferred.

COMPENSATION

The pay range for this role begins at $24.00 per hour and goes to $28.00 per hour depending on experience and skill sets.
